Assessment, Care Plans and Care Management
We conduct an essential assessment to evaluate a client’s safety, medical concerns, medication, and financial, legal and social issues. We then create a Care Plan from the assessments, in consultation with our client and family members as appropriate. Then each client is assigned a complex care team consisting of a Nurse Care Manager, Care Coordinator, and Social Worker.
After that, this team becomes the eyes and ears for your family. We manage all the details for your loved one, such as supervising home care services and making sure medical appointments are kept, and of course we quickly alert you to any changes or concerns that come up.
Crisis Management
Sometimes bad things happen fast: an unexpected diagnosis of dementia or Alzheimer ’s disease; broken hip; hospitalization; a lifetime of savings suddenly gone. We can swiftly move into action for you.
Referrals to Reputable Professionals
We refer you to other professional services based on your needs, and on our confidence and experience with those professional services. Unlike many other companies, we do not accept referral fees, kick-backs or any other type of compensation for referring to professional services. Our loyalty is strictly to our clients.
Elder Abuse and Financial Exploitation
Sadly, as our population ages, crimes against the elderly are increasing. Aging Solutions has deep experience identifying such situations, and what are the best approaches for handling them. We will work with your family when referrals to law enforcement and other regulatory agencies are necessary.
