Frequently, we conduct group interviews and consultations with family members who are scattered across several time zones using a conference call. On many occasions families have arranged for our staff to travel to where their loved ones are to conduct all necessary evaluations and assessments. In a situation where travelling is not an option, we can usually find local resources and help coordinate care long-distance.

What is a Care Plan?

A Care Plan is an action plan for future care based on the results of our assessment and is essential. We will identify problems, make recommendations to take certain actions, which may  include referring you to other types of help or services that may help your situation. We refer you to other professional services based on your needs, and our confidence and experience with those professional services.

Are you affiliated with other aging services providers, such as nursing homes or board and care homes who give you money to refer customers to them?

Absolutely not. We are an independent for-profit corporation whose sole interest is in serving our clients and families. Unlike many others, we do not accept referral fees, kickbacks or any other type of compensation for referring to professional services. Our loyalty is strictly to our clients.

Our parents refuse help and say they don’t want strangers in their home. Will you be able to convince him to accept assistance?

Many people initially resist help, despite the best intentions of their caring adult children. Oftentimes, having a professional assist them in making choices for themselves will make the difference.  ASI has the sensitivity and skills to help your parents break through this resistance. Our approach is to advise them of the options available to them, help them understand repercussions based on choices made and then to allow them to make their choices.  Our success rate is extremely high.

Aging Solutions, Inc. provides Comprehensive Client Centered Care Management through a consistent team of professionals who are not caregivers.

A professional geriatric care manager’s comprehensive assessment encompasses the total picture: medical, social, nutritional, emotional, financial, legal, and environmental. A Care Manager should be able to assess, coordinate and assist the client and family with all aspects of care over the long term and be able to respond to the needs of a client on a daily and weekly basis. Medical care should be part of this oversight as needed. At Aging Solutions, our experience is that most companies who say they provide comprehensive care management services charge for those extra services. Often, home care agencies do provide a level of care management at no charge, but this is typically limited to a form of supervision of caregivers. If it sounds like a home care or other agency is offering free care management services we suggest that you ask about the level of assessments they conduct to ensure that your relative’s needs will be fully met. For example, ask if a nurse is involved; or ask if they conduct daily and weekly social and medical care management and troubleshooting meetings on an on-going basis and for the long-term.
